Chapter 2 - The Matriarch's Command

The automatic double doors of the ambulance bay burst open with explosive force. Paramedics charged forward in a tight wedge, shoving a heavy hydraulic gurney across the linoleum floor.

"Systolic is down to sixty-five!" a paramedic shouted over the mechanical hiss of an automated chest compressor. "He went into PEA arrest two blocks away; we got ROSC after one milligram of epi and an amp of bicarb, but the rhythm is decaying fast! Sinus tach with wide complexes!"

On the gurney lay Richard Reynolds. The man was completely unrecognizable from the composed, untouchable billionaire who had worn an Italian cashmere sweater at brunch the day before. His skin had turned an ashen, mottled gray. Cold sweat soaked his hair, plastering it against his pale forehead. His lips were cyanotic—a terrifying, plum-dark blue that indicated his brain and organs were starving for oxygen as blood pooled uselessly into the false lumen of his ripped artery.

"Move! Bay 1, straight through!" I shouted, matching the gurney's sprint, my fingers already pressed against his right femoral artery, feeling for a pulse. It was faint, thready, slipping away like a dying current.

Just behind the paramedics, the main public doors of the emergency entrance swung wide.

Barbara Reynolds materialized in the lobby like an avenging fury dressed in high fashion. She was draped in a tailored camel-hair coat that trailed behind her, clutching an Hermès Birkin bag against her ribs like a shield. Her immaculate silver-blonde bob was disheveled for the first time in her adult life, but the entitlement that had defined her existence remained intact.

Beside her was Daniel, pale as chalk, his eyes wide with unadulterated terror. He looked younger, shattered, stripped of his usual composure as he watched his father being wheeled toward the threshold of death.

"Richard! Let me see him!" Barbara shrieked, her voice cutting through the clinical chaos of the trauma center. She tried to push past the triage desk, only to be physically blocked by two burly hospital security guards and an intake nurse.

"Ma'am, you cannot enter the clinical zone," the triage nurse said firmly, holding up her hands. "The trauma team is actively resuscitating your husband."

"Do you have any idea who I am?!" Barbara erupted, her voice trembling with hysteria and profound, volcanic rage. "My husband is Richard Reynolds! He sits on the advisory council of this city! We give millions to institutions like this! Get your filthy hands off my coat!"

She shoved past the triage nurse, lunging toward Bay 1. Her diamond rings caught the harsh fluorescent glare. Daniel reached out, grabbing his mother’s arm, his voice breaking with sheer anguish.

"Mom, stop! Let them work! They’re trying to save him!" Daniel begged, tears welling in his bloodshot eyes. "Mom, please!"

"I will not let some third-rate night-shift technicians murder your father in a public slaughterhouse!" Barbara screamed, scanning the room desperately. Her gaze caught a young, terrified-looking first-year resident holding a clipboard. She grabbed the lapels of his white coat with claw-like fingers.

"Listen to me, boy!" she spat, her face twisted in desperate, arrogant fury. "I don’t want residents! I don’t want interns! And I certainly don't want nurses touching him! I want the head of this hospital! I want the absolute best thoracic surgeon in the country! Get me the Chief of Surgery! *Right now!*"

The entire trauma reception area froze. Orderlies, nurses, and registration clerks turned their heads, their expressions shifting from irritation to sheer bewilderment.

The young resident gently removed her trembling hands from his coat, his eyes darting nervously toward the swinging doors of Trauma Bay 1.

"Ma'am," the resident whispered, swallowed by a strange mixture of awe and pity. "The Chief of Surgery is already inside."
